The below form must only be completed by an official club representative if a Queensland Government or Queensland Health official has requested access to attendance data at your club/venue, or if there is a confirmed COVID case or link to a positive case at your club/venue.
According to amendments to the Queensland Government’s COVID Safe Plans for the sport, recreation and fitness industry, clubs must be able to provide contact information of those in attendance within an hour if requested.
Clubs and venues must complete the below form to request access to this information.
